2024-25 Re-Enrollment

We look forward to receiving your re-enrollment forms for the 2024-25 school year. The forms below can be completed in one of two ways:

  • Save the PDF-fillable form to your computer, open and complete forms, resave, and then email to Cindy Potts in Admissions.
  • Or, print the pages, complete by hand, and deliver to the Admissions/Business Office.

The re-enrollment process is not complete until the registration fees have been paid and the Registration Form, the Enrollment Contract, the Tuition Collection Policy, the Church Involvement form, and the Student Covenant (6th-12th grades only) have been signed and received.
